One of the earliest things you can do in your Baldur’s Gate 3 game is speak to a sentient brain. I guess most human brains are sentient. But this one is, uh, a little different. And it will beg you to release it from its skull cage. But is that the best idea? Should you free the brain in Baldur’s Gate 3?

So, you’ve woken up on a crashed Mindflayer’s ship. There’s a weird tadpole swimming around in your head, and a distressed brain is demanding you help it. This campaign is starting off on a weird foot, but things aren’t always as they appear.
The brain can actually be a very helpful companion in the fights to come if you choose to free it. But to do so, you’ll need to pass a series of skill checks. Here’s what to expect. And note, some of your options might be different, depending on your character build.
The correct answers will be bolded.
Dialogue Option 1
- Who am I talking to – a man or a brain?
- I think you’re past the point of saving. Tell me what to do.
- You sound afraid. Why?
- Leave.
Dialogue Option 2
- I think you’re past the point of saving. Tell me what to do.
- You sound afraid. Why?
- Destroy the brain.
- Leave.

Dialogue Option 3
- [INVESTIGATION] Inspect the exposed brain.
- [STRENGTH] Break the skull.
- [DEXTERITY] Gently prise the brain from the skull.
- Destroy the brain.
- Leave.
All of these options will require you to land at least a 10. Pass, and you’ll move on to the next phase.
Dialogue Option 4
- [DEXTERITY] Mutilate the brain.
- Spare the creature – any injuries might weaken it.
You’ll either need to pass the Dexterity check or spare the creature to turn it friendly. Doing so will add it to your party… at least if you don’t choose to attack it in the next dialogue choice. And this Thrall is very useful to have!
While it suffers from a disadvantage to attacks due to its newborn status, it has 21 HP (making it beefier than any spellcaster), and the attack Claw, which does between 4-10 damage. Nice!
