Infernal Spires have corrupted the Ruins of Qara-Yisu Stronghold in Diablo 4 and now it’s up to you and your allies to cleanse the area of the demonic presence. This stronghold is full of false statues and classic demon enemies that put up plenty of resistance, but nothing can stop you when you know exactly how to clear out the ruins.
Where is the Ruins of Qara-Yisu Stronghold in Diablo 4?
You can find the Ruins of Qara-Yisu Stronghold in the southeast section of the Dry Steppes Region. The Dry Steppes is the third major region in Diablo 4 and is located on the western side of the map. This stronghold is located on the border of Kehjistan and the Dry Steppes.
I used the Hidden Overlook Waypoint as the fastest route to the stronghold in the Dry Steppes. This Waypoint is unlocked by completing some of the main quests in this area and is a great way to reach the ruins. When you reach the ruins entrance, you will see the first set of statues that come alive and you can enter after clearing these.
How to Find and Destroy the Infernal Spires in the Ruins of Qara-Yisu
Three different Infernal Spires are scattered around the ruins and are marked with red skulls on the map. You simply need to break each of the spires with any kind of damage. There are no tricks involved with this section of the stronghold, just plenty of enemies.
As you try to destroy each of the Infernal Spires, more demons will spawn. These need to be dealt with or they will quickly pile up and potentially overwhelm you. About halfway through the life on each spire, some of the larger Balrog demons will appear, along with various elites to make matters worse.
However, as long as you manage the enemies before dealing too much more damage to the Infernal Spires in the Ruins of Qara-Yisu Stronghold, then reaching the final boss will be a breeze compared to some of the other major dungeons.
How to Defeat Utulka the Voice Below in Diablo 4
Utulku the Voice below acts like other Shaman-type demons and relies heavily on casting spells. Close the gap on this boss where you can and avoid the electric attacks that are left on the ground. If you can avoid the AOE attacks on the ground, the rest of the fight gets much easier.
Throughout the entire fight, Utulku really only uses electric bolts and the AOE ground attack. Some minions will spawn as well, but they can be quickly dispatched. With each tick that goes down on Utulku’s health bar, more electric pools will spawn when he casts them. So evading attacks will get more difficult as the fight comes to a close.
Burning Utulku the Voice Below is much easier to pull off when compared to other bosses though. Ensure that you have your ultimate ready to go in the last leg of the fight. I had my shadow clone on my Rogue character, along with full imbuements ready to go so I didn’t have to deal with the extra AOE for long in the Diablo 4 Ruins of Qara-Yisu Stronghold.
Ruins of Qara-Yisu Rewards
When you defeat the final boss, you will be rewarded with 100 Renown and some rare gear. After I was done with the Stronghold, I earned a Legendary bow and the chances of decent gear are much higher in a tougher dungeon.

After you finish the Stronghold, the area will gain a Waypoint and a couple of side quests will appear at the Ruins of Qara-Yisu. One of the quests is standard but the other can get fairly confusing. To make sure you don’t get stuck on Blood and Sweat, make sure to check out my full guide.
